Coalition Coalitions Pro Homeless Puerto Rico, Inc.

It is a non-profit organization that since 2003 has served as project coordinator housing and services for homeless agency in 54 municipalities of the island. Among its projects are the HMIS connection Boricua, Coordinated Entry System and the Integrated Alternative Clinic. Our facilities are located in Ponce and Caguas.

Projects

The Project HMIS connection Boricua it is a requirement of the Department of Housing and Urban Development (HUD) which provides a technological basis for projects that provide services to homeless individuals and families maintain an electronic record of data people they serve.

The Coordinated Entry System (CES for its acronym in English), is a system designed in compliance with the regulation of Continuing Care Systems, that, coordinated and unified manner, assesses the needs of homeless individuals and families, including those fleeing domestic violence situations, support services and location in alternative housing available according to your needs.

First specialized clinic that integrates primary care services, psychological services, substance abuse treatment assisted with drugs, counseling, social work, support services and home location.

The initiative for the implementation of the model Housing First is aimed at ensuring the basic human right of every citizen to decent and worthy housing, without conditions of success in treatment or withdrawal, respecting their right to choose their housing, while ensuring the availability of support services necessary for their stability and community life.
